OCCUPATIONAL THERAPY BILLING, EXPLAINED
Occupational therapy billing services manage insurance claims and reimbursement for OT evaluations, therapeutic activities, self-care training, rehabilitation services and other covered treatments.
Unlike routine medical billing, OT claims often involve time-based procedure codes, therapy-specific modifiers (GO, CO and KX), provider qualification requirements, treatment documentation and insurance authorization limits. A mistake in any of these areas can delay payment for services your therapists have already delivered.
SkyLine MB manages the process from eligibility verification to payment reconciliation, with attention to the coding and insurance requirements that affect OT reimbursement.

Built for occupational therapy practices
Billing support designed around documentation, therapy units, Medicare rules and reimbursement.
SKYLINE MB / OT BILLING
Every occupational therapy practice works differently. A pediatric OT clinic may handle Medicaid authorizations and developmental therapy services, while an outpatient rehabilitation center may manage Medicare claims, hand therapy and post-injury rehabilitation. Your billing process should reflect those differences.
At SkyLine MB, we provide occupational therapy medical billing services for independent occupational therapists, outpatient rehabilitation clinics, pediatric therapy centers and multi-provider practices. With 10 years of medical billing experience, we help manage insurance verification, charge review, claims submission, payment posting, denial management and A/R follow-up.
Our team pays attention to the details that commonly cause problems — therapy modifiers, timed treatment units, authorization requirements and incomplete claim information — and works within your existing administrative process.
Your therapists focus on helping patients achieve functional independence. We help your practice collect the payments it has earned.
WHAT WE HANDLE
Nine connected services covering the OT revenue cycle, from insurance verification to outstanding payment follow-up.
01
Coverage problems can turn a completed OT session into an unpaid claim. We verify active coverage, OT benefits, patient responsibility, referral requirements and treatment limitations.
Deductibles, copayments and plan-specific rules are reviewed so coverage questions are answered before they delay billing.
02
An expired authorization or exceeded visit allowance can interrupt reimbursement for ongoing therapy.
We track authorization periods, approved services and remaining visits, and coordinate reauthorization information with your administrative and clinical staff.
03
We review OT CPT codes, documented treatment time, billable units, ICD-10-CM codes and therapy modifiers — see the OT CPT code table.
Charge review covers evaluations, therapeutic exercise, neuromuscular re-education, therapeutic activities and self-care training.
04
A claim should be checked before submission and monitored afterward. We prepare electronic OT claims with the required patient, provider, procedure and insurance details.
Rejected claims are investigated and corrected for resubmission.
05
Common denial reasons include missing GO modifiers, authorization problems, eligibility issues, coding discrepancies and insufficient documentation.
We review payer responses, coordinate corrections or appeals and monitor recurring denial patterns.
06
Your clinic may be treating patients every day while older claims remain unpaid.
We track outstanding balances, investigate aging claims and prioritize follow-up by payer response and filing requirements.
07
Knowing what you’ve billed isn’t the same as knowing what you’ve collected. We post payments from ERAs and EOBs, review contractual adjustments and reconcile balances.
Possible underpayments are flagged for follow-up.
08
Adding a therapist or expanding your practice creates enrollment work.
We assist with credentialing administration, payer applications, CAQH information, provider updates and application follow-up. Network participation remains subject to each payer’s approval.
09
We support patient statements, balance tracking and billing inquiries within the agreed scope.
Reports cover submitted claims, insurance payments, denials, adjustments and outstanding balances so you can set clear priorities.

MEDICARE OT BILLING UPDATES
Outdated billing information leads to avoidable denials and incorrect reimbursement. These are the rules we check on every Medicare OT claim.
For calendar year 2026, Medicare’s KX modifier threshold for occupational therapy is $2,480.
Above it, KX confirms continued services are medically necessary and documented. It is not an automatic cap on therapy.
Most OT treatment codes bill in 15-minute units. Medicare totals the qualifying timed minutes in a visit to decide billable units.
We check minutes and units against the treatment note.
When an occupational therapy assistant furnishes part of a service, Medicare may require the CO modifier.
Plan-of-care certification and documentation must support every billed visit.
Based on CMS outpatient therapy guidance (Medicare Benefit Policy Manual Ch. 15; Claims Processing Manual Ch. 5) and the CY 2026 Physician Fee Schedule. Commercial payers may apply different rules.
OT MODIFIERS & BILLING REQUIREMENTS
Modifiers identify the therapy discipline, assistant involvement or medical necessity. We review them before submission so each claim reflects the documented treatment and payer rules.

| Modifier | What it indicates | Common billing concern |
|---|---|---|
| GO | Services under an occupational therapy plan of care | Missing on claims that require an OT therapy modifier |
| CO | Services furnished in whole or in part by an occupational therapy assistant | Incorrect reporting of OTA participation |
| KX | Medical necessity requirements met at or above the therapy threshold | Missing when required, or used without supporting documentation |
| 59 | Distinct procedural service, when applicable and supported | Incorrect unbundling or insufficient documentation |
| 96 | Habilitative services, when required by the payer | Habilitative care reported without a required modifier |
| 97 | Rehabilitative services, when required by the payer | Modifier not aligned with payer instructions |

WHO WE SERVE

We adapt billing responsibilities around your services, payer mix, treatment volume and administrative needs.
COMMON CHALLENGES
Your clinic can deliver excellent care and still feel financial pressure from avoidable billing problems.
Time-based OT procedures need accurate minute calculations. Inconsistent documentation or wrong units create claim issues.
Medicare and other payers may require specific modifiers for OT services.
Some plans restrict covered visits or require authorization for continued treatment.
Claims may be questioned when notes don’t support the service, time or medical necessity.
Government programs have requirements that differ by program and sometimes by state.
Even properly submitted claims may stall when payers request more information.
AUTHORIZATIONS & VISITS
Few things are more frustrating than finishing a visit and discovering the authorization had already expired. We help your team compare three records:
Comparing these details catches discrepancies before they become avoidable claim problems. When follow-up is needed, we coordinate with your administrative team.

IN-HOUSE VS. OUTSOURCED
If your staff spends too much time correcting claims, tracking authorization limits or calling insurance companies, outsourced occupational therapy billing may provide useful support.

Request an occupational therapy billing review to see whether outsourcing makes sense for your clinic.
