PT BILLING, EXPLAINED
Physical therapy billing services manage the reimbursement side of outpatient rehabilitation: insurance eligibility and authorization checks, PT coding and charge review, electronic claim submission, denial follow-up, payment posting and accounts receivable management.
PT billing is different from general medical billing because most treatment codes are time-based 15-minute units, Medicare applies the 8-minute rule and therapy modifiers (GP, KX, CQ), and patients often have visit limits or authorizations that run out mid-plan of care. Claims must match documented minutes, the plan of care and the payer’s rules — or they deny.

SKYLINE MB / PHYSICAL THERAPY BILLING
You can see patients all day and still struggle with unpaid insurance claims. Maybe treatment units are being billed incorrectly. Maybe authorizations are running out without anyone noticing. Or perhaps claims have been sitting in accounts receivable long after the treatment was completed.
For physical therapy practices, these issues can turn a productive month into a frustrating financial one.
At SkyLine MB, we provide physical therapy billing services that help outpatient PT clinics manage claims, address reimbursement delays and stay on top of outstanding payments. Backed by 10 years of medical billing experience, we support the revenue cycle from insurance eligibility and authorization checks to PT coding, electronic claim submission, denial follow-up and payment posting.
We help you connect the work happening in your treatment rooms with the payments your clinic receives. Our support covers the details that affect reimbursement — service documentation, timed treatment units, modifiers, insurance requirements and outstanding claims.
Instead of a submit-and-forget billing process, we maintain an organized workflow that follows claims through resolution. You focus on helping patients regain strength, mobility and independence. We’ll help manage the billing work behind every visit.
WHAT WE HANDLE
From the patient’s first evaluation to the final insurance payment, every stage of your revenue cycle deserves attention.
01
Start treatment with a clearer picture of the patient’s coverage. We help verify coverage status, physical therapy benefits, copayments, deductibles, coinsurance, and applicable visit or authorization requirements.
Our goal is to help your practice address potential insurance problems before they interrupt reimbursement.
02
Physical therapy patients often need several appointments over the course of treatment. A missed authorization or exhausted visit allowance can create denials and delays.
We help monitor approved visits, authorization information, effective dates and reauthorization needs — and coordinate with your team when action is required.
03
Every billed unit should match the treatment documented. We support coding and charge review for evaluations, re-evaluations and timed treatment codes such as 97110, 97112, 97116, 97140, 97530 and 97535 — see the PT CPT code table.
We aim to reduce billing errors without changing or overstating the treatment your clinicians performed.
04
Outpatient therapy claims require attention to rules that may not apply to every commercial plan.
We support claim reviews involving the Medicare 8-minute rule, time-based unit calculations, documentation requirements and the use of modifiers such as GP, KX and CQ when required by current Medicare policy.
05
Submitting a claim is the beginning of the follow-up process, not the end. We review billing information, prepare electronic claims and track submission responses.
When a clearinghouse or payer rejects a claim, we investigate and coordinate corrections so unresolved submissions don’t disappear into an aging queue.
06
Denied PT claims may involve coding errors, missing modifiers, insufficient authorization, coverage limits, medical necessity or provider information discrepancies.
We investigate the denial reason, help prepare corrections or appeals when appropriate, and identify recurring denial patterns affecting multiple claims.
07
Every outstanding balance needs an owner and a next step. We track unpaid claims, review aging balances and prioritize accounts by claim status and filing requirements.
We contact payers when necessary, document follow-up activity and move eligible balances toward resolution.
08
Make sure your financial records reflect what was actually paid. We post insurance payments from ERAs and EOBs, review adjustments and reconcile account balances.
Potential underpayments are flagged for further investigation, giving a more accurate picture of reimbursement across your clinic.
09
Adding another therapist? Provider enrollment can affect when and how claims are processed.
We assist with credentialing administration, payer enrollment follow-up, CAQH information and provider record updates for individual and group enrollment.
10
Clear payment information helps you make better decisions. We support patient balance tracking, statement preparation and billing communication within the agreed scope.
Reporting covers claim activity, payments, denials and outstanding insurance balances — without you investigating every account.
11
For Medicare patients, the plan of care must be certified by the physician or NPP, and progress reports are required at regular intervals.
We help track certification and recertification dates and flag missing signatures before they put a run of visits at risk.
12
Work-injury and auto-accident patients bring claim numbers, adjusters, fee schedules and extra documentation.
We help organize claim information, submit to the correct carrier and follow up so these visits don’t stall behind commercial claims.
PT CPT CODES
Common outpatient PT codes, whether they are timed, and the billing detail that most often causes denials.
| CPT code | Service | Timed? | Billing watch-point |
|---|---|---|---|
| 97161 / 97162 / 97163 | PT evaluation — low, moderate, high complexity | Untimed | Complexity must match documented history, exam and presentation |
| 97164 | PT re-evaluation | Untimed | Needs a documented change in condition or plan of care |
| 97110 | Therapeutic exercise | 15 min | Units must match documented minutes |
| 97112 | Neuromuscular re-education | 15 min | Documentation should show balance, coordination or proprioception goals |
| 97116 | Gait training | 15 min | Distinguish from therapeutic activities |
| 97140 | Manual therapy techniques | 15 min | NCCI edits with some evaluation and treatment codes |
| 97530 | Therapeutic activities (dynamic, functional) | 15 min | Watch NCCI pairing with 97150 and evaluations |
| 97535 | Self-care / home management training | 15 min | Functional goal must be documented |
| 97750 | Physical performance test or measurement | 15 min | Requires a written report |
| 97150 | Group therapeutic procedures (2+ patients) | Untimed | Bill once per session, not per unit |
| 97010 / G0283 / 97014 | Hot-cold packs / unattended electrical stimulation | Untimed | 97010 is bundled by Medicare; Medicare uses G0283 for unattended e-stim |
| 97760 / 97761 | Orthotic management / prosthetic training | 15 min | Device documentation needed |

MEDICARE TIMED-UNIT RULES
Your therapists track every minute. Your billing should too — because the way minutes become units decides what gets paid.
Medicare adds up the total minutes of timed services in a visit. 8–22 minutes = 1 unit, 23–37 = 2 units, 38–52 = 3 units, 53–67 = 4 units, and so on.
Remaining minutes are assigned to the service with the most time.
Evaluations (97161–97164), group therapy (97150) and supervised modalities are untimed — billed once regardless of minutes.
Only timed treatment codes count toward the 8-minute calculation.
GP identifies services under a PT plan of care. KX confirms medical necessity once annual therapy spending passes the Medicare threshold.
CQ marks services furnished in whole or in part by a PTA, which Medicare pays at a reduced rate.
Based on CMS Medicare Benefit Policy Manual (Ch. 15) and Medicare Claims Processing Manual (Ch. 5). Commercial payers may count units differently.
PT MODIFIERS
Most PT denials trace back to a handful of modifiers. We check every claim against Medicare rules and each payer’s own policy before it is submitted.

| Modifier | When it applies | Common billing risk |
|---|---|---|
| GP | Service delivered under an outpatient physical therapy plan of care | Missing on Medicare and many commercial claims |
| KX | Medical necessity documented after the annual Medicare therapy threshold is reached | Applied without supporting documentation |
| CQ | Service furnished in whole or in part by a physical therapist assistant | Not applied when a PTA provided part of the minutes |
| 59 / XE / XS / XP / XU | Distinct service to bypass an NCCI edit (e.g., 97140 with 97530) | Overuse is a frequent audit target |
| 76 / 77 | Repeat procedure on the same day | Denied as a duplicate without the modifier |
| 95 / POS 02 or 10 | Covered telehealth therapy services | Service or payer not eligible for telehealth |
| GN / GO | Speech-language or occupational therapy plans of care | Mixed with GP on the same claim line |

WHO WE SERVE
Not every rehabilitation practice has the same patient population, schedule or insurance mix. We help organize revenue cycle workflows for different physical therapy settings:
Whether you’re managing one treatment room or several clinic locations, your billing process should support your growth.
WHERE REVENUE LEAKS
Your clinic shouldn’t have to choose between managing patients and managing payments.
Therapeutic exercise, manual therapy and other time-based services require accurate unit calculations. Too many or too few units create payment issues or compliance risks.
Rehab plans need multiple visits, while approved visit counts and coverage restrictions complicate billing.
Medicare outpatient therapy claims involve specific modifiers, documentation, therapy thresholds and assistant payment rules.
A rejected claim becomes an unpaid balance when nobody has time to investigate it.
Staff schedule evaluations, manage appointments and collect payments — insurance follow-up shouldn’t consume the rest of their day.
Unsigned certifications or overdue progress reports can put a whole run of Medicare visits at risk.
AUTHORIZATIONS & VISIT LIMITS
Most PT denials start long before a claim is submitted. For every visit we compare three records so problems are caught while they can still be fixed:
When the records don’t match, your team is alerted before the next visit — instead of finding out from a denial weeks later.

WHY SKYLINE MB
We bring 10 years of experience to claim processing, insurance follow-up and revenue cycle challenges facing healthcare practices.
Timed treatment units, CPT codes, modifiers, authorizations and payer-specific requirements get the attention they need.
Claim preparation, payment posting, denial management, outstanding A/R and revenue cycle reporting in one workflow.
We build responsibilities around your practice rather than expecting every clinic to follow the same process.
Know which claims are paid, which are pending and where additional follow-up is required.
Less administrative billing pressure lets your staff focus on scheduling, patient service and clinic operations.
KEEP YOUR WORKFLOW
You’ve already invested in your practice management system, documentation processes and staff training. A new billing arrangement shouldn’t mean starting over.
We begin by understanding how your team handles appointments, documentation, charges, payments and insurance follow-up — then agree which responsibilities make sense for SkyLine MB to manage.
Your clinic may benefit from additional support if patient volume is rising but collections aren’t, staff keep correcting units or modifiers, authorization issues affect payments, Medicare requirements create uncertainty, or aging A/R keeps growing.
Need end-to-end PT billing? We review the entire workflow. Only need help with insurance claims and aged A/R? We can discuss a focused arrangement.

SkyLine MB can discuss full-service or selected support based on your practice’s needs.
Yes. We support outpatient physical therapy claims involving Medicare requirements, including therapy modifiers such as GP, KX and CQ, timed unit calculations and documentation-related billing checks.
The Medicare 8-minute rule determines billable units for time-based outpatient therapy services. Total timed minutes are added up: 8–22 minutes is one unit, 23–37 minutes two units, 38–52 minutes three units, and so on. Not every commercial insurer follows the same methodology.
Common physical therapy codes include 97110 for therapeutic exercises, 97112 for neuromuscular re-education, 97116 for gait training, 97140 for manual therapy and 97530 for therapeutic activities, along with evaluation codes 97161–97163 and re-evaluation 97164. Other codes may apply depending on the documented services.
GP indicates a service delivered under a physical therapy plan of care. KX confirms that services above the annual Medicare therapy threshold are medically necessary and documented. CQ identifies services furnished in whole or in part by a physical therapist assistant.
Yes. Our denial management support includes reviewing payer responses, identifying billing-related errors, coordinating corrections and following up on claims or appeals when appropriate.
We can review your existing EHR or practice management platform during consultation. Workflow compatibility, access arrangements and technical requirements depend on your system and agreed services.
Yes. We can discuss outsourced billing arrangements for independent physical therapists, small outpatient clinics and larger rehabilitation practices.
Pricing depends on your claim volume, number of providers, payer mix and required billing services. SkyLine MB provides a quote after reviewing your clinic — there is no cost for the initial consultation.
We can review outstanding accounts and determine which balances may be appropriate for follow-up, correction or appeal. Recovery depends on documentation, payer policies, filing deadlines and claim eligibility.
