PEDIATRIC BILLING, EXPLAINED
Pediatric medical billing services manage the claims and payments behind children’s healthcare: eligibility checks, newborn coverage, well-child and sick-visit coding, vaccine and immunization billing, Medicaid and CHIP claims, denial management, payment posting and A/R follow-up.
Pediatric billing is different from adult primary care because age-specific preventive codes change as children grow, vaccine product and administration are billed separately (with VFC rules for state-supplied vaccines), sick and well visits on the same day need modifier 25, and a large share of patients are covered by Medicaid, CHIP or managed care.

SKYLINE MB / PEDIATRIC BILLING
Between newborn appointments, immunizations, preventive screenings, sick visits and follow-up care, your team handles a demanding schedule. But behind those appointments, billing problems can quietly build up.
A vaccine administration charge gets overlooked. A newborn’s coverage hasn’t been updated. A preventive visit is billed incorrectly. A denied Medicaid claim remains untouched for weeks. Individually small, together these issues put pressure on your cash flow.
At SkyLine MB, we help pediatric providers manage the billing challenges that interfere with collections and consume valuable staff time. From immunization claims and preventive care billing to Medicaid follow-ups and denial management, our pediatric medical billing services keep your revenue cycle organized.
With 10 years of medical billing experience, we support independent pediatricians, growing clinics and group practices across the United States — bringing consistency to claim preparation, insurance verification, payment tracking and outstanding balance follow-up.
The result is a more manageable administrative workload and better visibility into the revenue you’ve already earned.
WHAT WE HANDLE
You need a billing partner that supports the work before and after a claim goes to the insurance company.
01
Don’t discover coverage problems after the appointment. We verify eligibility, subscriber information, plan benefits and patient responsibility before claims are processed.
Our goal is to catch preventable insurance issues earlier and reduce administrative rework.
02
We support claims for age-appropriate wellness visits, preventive screenings, developmental assessments and other documented pediatric services.
Attention goes to the right age-based CPT code, modifiers and payer guidelines — see the pediatric CPT code table.
03
Vaccine billing deserves more than a quick charge entry. We cover vaccine product charges, administration services and counseling documentation.
Vaccines for Children (VFC) program requirements are applied where relevant, and inconsistencies that delay payment are flagged.
04
A newborn’s first visits shouldn’t create months of insurance follow-up.
We review newborn coverage, dependent enrollment, subscriber relationships and claim requirements that differ by plan — so early visits aren’t stuck waiting for coverage updates.
05
Different payers, different requirements, one organized workflow. We support claims for Medicaid, CHIP, managed care organizations and commercial plans.
We review payer-specific requirements, track claim statuses and follow up on outstanding reimbursements.
06
Small coding mistakes shouldn’t repeatedly interrupt collections. We review charges for E/M visits, preventive medicine, vaccinations, screenings and other procedures.
Our approach considers the documented service, coding guidelines, medical necessity and payer policies.
07
Getting a claim out the door isn’t enough — it needs to move through the payer’s process.
We prepare and submit electronic claims, check for errors, investigate rejections and coordinate resubmission within filing deadlines.
08
A denied claim shouldn’t automatically become lost revenue. We investigate denial reasons, organize supporting information and pursue corrections or appeals where appropriate.
We also look for recurring denial patterns you can fix upstream.
09
Your outstanding revenue needs attention, not another spreadsheet. We monitor unpaid balances, review aging accounts and follow up with payers.
You get a clearer view of outstanding payments and what’s affecting collection.
10
Know what’s been paid, what’s pending and what still needs attention. We support payment posting, adjustment reconciliation and underpayment review.
Billing performance reports help you make informed operational decisions.
11
Adding a pediatrician or opening another location? We assist with enrollment activities, CAQH information, payer credentialing follow-up and provider record updates.
Medicaid and managed care enrollment is tracked until each provider is active.
12
Family plans, divorced-parent coverage and secondary insurance make pediatric patient balances confusing.
We support clear statements, guarantor details and billing questions from parents within the agreed scope.
PEDIATRIC CPT CODES
Common preventive, newborn, vaccine and screening codes — and the billing detail that most often causes denials.
| CPT code | Service | Billing watch-point |
|---|---|---|
| 99381–99385 | Preventive visit, new patient (infant to age 17) | Code is chosen by the child’s age |
| 99391–99395 | Preventive visit, established patient (infant to age 17) | Age band must match date of service |
| 99460 / 99461 / 99462 / 99463 | Newborn care — initial hospital, other setting, subsequent day, same-day discharge | Newborn’s own ID or mother’s coverage rules |
| 90460 / +90461 | Vaccine administration with counseling (through age 18) — first / each additional component | Counseling by physician or QHP must be documented |
| 90471–90474 | Vaccine administration without counseling (injection / oral or intranasal) | Not billed with 90460 for the same vaccine |
| Vaccine product codes (e.g., 90700, 90707, 90713) | The vaccine itself | VFC stock billed per payer rules, often $0 or with SL |
| 96110 | Developmental screening with a standardized instrument | Per instrument; payer frequency limits |
| 96127 | Brief emotional / behavioral assessment | Per standardized instrument |
| 96161 | Caregiver-focused health risk assessment (e.g., maternal depression screening) | Billed under the infant’s claim |
| 99173 / 92551 | Visual acuity screening / hearing screening | Separately billable for many payers |
| 99211–99215 + modifier 25 | Sick (problem-oriented) visit on the same day as a preventive visit | Needs separately documented problem work |
Simplified descriptors for reference. CPT® is a registered trademark of the American Medical Association. Medicaid and CHIP rules differ by state — we verify current payer and program requirements before claims are submitted.
VACCINE BILLING RULES
Immunization visits combine products, administration codes, counseling and program rules — small details decide whether they’re paid.
VFC vaccines are supplied free, so the practice bills the administration (with the state’s VFC admin-fee rules) and reports the product as each payer requires.
Private-stock vaccines are billed with both the product and administration codes.
90460 covers the first vaccine component when a physician or QHP counsels the patient or family (age 18 and under); 90461 adds each extra component.
Without that counseling, administration bills with 90471–90474.
When a significant problem is addressed during a well-child visit, the problem-oriented E/M can be billed with modifier 25.
The note must show the separate problem work — otherwise it’s denied as part of the preventive visit.
Based on AMA CPT® guidelines, CDC Vaccines for Children program rules and state Medicaid billing guidance. Requirements differ by state and payer.
PEDIATRIC MODIFIERS
Most pediatric denials come from same-day services, vaccine reporting and Medicaid program rules. We check every claim against the payer’s own policy before it is submitted.

| Modifier | When it applies | Common billing risk |
|---|---|---|
| 25 | Significant, separately identifiable E/M on the same day as a preventive visit or procedure | Sick visit denied without separate documentation |
| 33 | Preventive service that should be covered without cost-sharing | Patient wrongly charged a copay |
| SL | State-supplied (VFC) vaccine, required by some Medicaid programs | Missing on VFC doses where required |
| EP | Services provided under the Medicaid EPSDT program, where required | EPSDT visit not identified |
| 59 / XE / XS / XP / XU | Distinct service to bypass an NCCI edit (e.g., some screenings) | Overuse is a frequent audit target |
| 95 / POS 02 or 10 | Covered telehealth visits | Service or payer not eligible for telehealth |
| 52 | Reduced service (e.g., screening not fully completed) | Billed in full when only part was performed |

WHAT WE BILL
Pediatric billing isn’t the same as billing for an adult primary care office. Your revenue cycle must account for age-specific services, family insurance arrangements, preventive schedules and immunizations. We support:
We make sure the billing workflow reflects the services your pediatric team actually provides.
COMMON CHALLENGES
Your practice deserves a billing process that can keep up with your patient schedule.
Immunization visits involve products, administration, counseling and program rules — one missing detail can cause rejection.
Preventive visits, screenings and same-day problem services create coding complications.
Eligibility changes, managed care rules and enrollment discrepancies turn routine claims into repeated follow-up.
When staff are busy with appointments and calls, unpaid claims fall behind.
Babies are often seen before they’re added to a parent’s plan or Medicaid.
Two parents’ plans, the birthday rule and custody arrangements decide which plan pays first.
CLAIM TRACKING
Think about how much time your staff spends on unpaid claims, billing questions and rejected charges. With SkyLine MB supporting your revenue cycle, every visit is followed through three checkpoints:
The result: more time for patient care, fewer unresolved billing issues, better visibility into outstanding revenue and less pressure on your front desk.

ALREADY HAVE A BILLING TEAM?
Not every pediatric practice needs to replace its existing billing department — sometimes the right solution is additional support.
Your front desk may be strong but struggle with insurance follow-ups. Your biller may handle daily claims well but need help clearing old A/R. Or you may be transitioning between billing systems.
SkyLine MB can discuss end-to-end outsourced pediatric billing, denial and A/R follow-up, eligibility and claim processing support, revenue cycle workflow review, or credentialing assistance.
You may benefit if staff spend hours on claim follow-up, vaccine and well-child payments are hard to reconcile, denials stay unresolved, or you’ve lost confidence in your current vendor.
