UNDERSTANDING ASC BILLING
Ambulatory surgical center billing services manage the administrative and financial processes for eligible outpatient surgical procedures performed in Medicare-certified or other appropriately licensed surgery centers.
Unlike physician office billing, an ASC facility claim represents covered facility services associated with a surgical procedure. The surgeon’s and anesthesiologist’s professional services are generally billed separately under their own arrangements.
Facility reimbursement can be affected by procedure eligibility, multiple-procedure rules, packaged items, implanted devices and payer contracts — and it must reflect what actually happened during the case, not just what was scheduled. SkyLine MB reviews case documentation, facility charges and insurance requirements before claims go out, and investigates balances that are delayed or lower than expected.

Every surgical case deserves a complete billing review
Connecting surgical documentation, facility charges, payer contracts and insurance payments.
AMBULATORY SURGERY CENTER REVENUE SUPPORT
Your OR team knows whether a procedure was completed successfully. But can your billing records show whether every eligible facility charge was captured and the case was reimbursed correctly?
A surgery may have needed extra supplies or an implant — billing still has to decide whether those items are packaged into the facility payment or separately reimbursable. A second procedure may need multiple-procedure review. An authorization may not match the final surgery. A contracted case rate may need verification.
SkyLine MB helps ASC administrators and billing teams answer those questions. With 10 years of medical billing experience, we review surgical documentation, claim information, reimbursement rules and payer responses — from the completed case through final account reconciliation.
You know how many cases your center performs. We help you see what happens to the revenue after they leave the OR.
ASC REVENUE CYCLE SERVICES
Your facility shouldn’t lose visibility into a case the moment the patient is discharged.
01
Outpatient surgery verification goes beyond an active policy.
We review benefits, provider participation, patient responsibility and coverage requirements for the planned procedure.
02
An approved procedure and a performed procedure must match.
We review approved procedure details, dates and facility information, and coordinate discrepancies when a case changes.
03
An OR record should translate into an accurate facility claim.
We review procedures, CPT/HCPCS codes, facility charges, modifiers, devices and multiple procedures — see the ASC procedure table.
04
Facility billing deserves a facility-specific review.
We prepare claims in the payer’s required format, check facility identifiers and covered services, and correct rejections.
05
High-value surgical denials shouldn’t sit unresolved.
We review coverage, authorization, eligibility, edit and documentation denials, and track recurring problems across cases.
06
Completed cases shouldn’t become long-term receivables.
We monitor unpaid ASC claims and prioritize follow-up by payer status and filing or appeal deadlines.
07
A payment posted isn’t necessarily a payment verified.
We reconcile adjustments and balances and, with contract data, check reimbursement against the agreed methodology.
08
Enrollment discrepancies create preventable claim problems.
We assist with facility and provider enrollment administration, information updates and payer follow-up.
09
See which cases generated revenue — and which still need attention.
Reports cover submitted cases, payments, denials, balances and discrepancies.
ASC PROCEDURE CODING
Reimbursement depends on the precise procedure, current CMS coverage and payment files and payer policy — so we show procedure categories, not a misleading list of universally payable codes.
| Surgical specialty | Common ASC procedures | Billing consideration |
|---|---|---|
| Gastroenterology | Colonoscopy, upper endoscopy and eligible therapeutic procedures | Multiple services, coding edits and payer coverage |
| Ophthalmology | Cataract surgery and eligible ophthalmic procedures | Procedure coding and device-related rules |
| Orthopedics | Arthroscopy, eligible joint procedures and selected orthopedic surgery | Implants, device policies, multiple procedures and authorization |
| Pain Management | Epidural injections, nerve procedures and selected interventions | Medical necessity, imaging guidance and bundling |
| Podiatry | Foot and ankle procedures | Laterality, procedure combinations and payer requirements |
| Urology | Cystoscopy and eligible outpatient urologic procedures | Covered procedure status and surgical coding |
| Gynecology | Hysteroscopy and eligible gynecologic procedures | Procedure combinations and coverage requirements |
| ENT | Selected ear, nose and throat procedures | Surgical coding, implants where applicable and coverage |
Inclusion in this table doesn’t mean a procedure is Medicare ASC-covered or separately payable. Each CPT/HCPCS code must be checked against the 2026 ASC-approved procedure and payment files for the date of service. CPT® is a registered trademark of the American Medical Association.
2026 MEDICARE ASC PAYMENT SYSTEM
Medicare doesn’t pay ASCs like physician offices. The ASC Payment System sets facility reimbursement for eligible procedures and covered associated services.
A procedure performed in an ASC isn’t automatically payable — the code must meet coverage and payment criteria for the date of service.
Many supplies are packaged; logging an item in the OR doesn’t make it separately billable.
Second procedures may receive multiple-procedure payment treatment based on codes, payment indicators and contract.
Device-intensive procedures follow specific Medicare policies, including discontinued cases and device credits.
Missing ASCQR Program requirements can reduce annual payment updates — a separate responsibility from claims, confirmed within scope.
CMS issues periodic updates (including October 2026); billing must use current procedure lists, HCPCS and payment indicators.
Based on the CMS ASC Payment System, ASC-approved procedure and payment files (Addenda AA/BB), Medicare Claims Processing Manual Ch. 14 and NCCI policy. Commercial contracts may differ.
SURGICAL MODIFIERS & PAYMENT RULES
The right modifier depends on the procedure, documentation, payer and claim type. Some modifiers used on professional claims aren’t required — or appropriate — the same way on ASC facility claims.

| Modifier | Common purpose | What requires review |
|---|---|---|
| 50 | Bilateral procedure, when applicable | Procedure- and payer-specific reporting rules |
| 51 | Multiple procedures, where required | Multiple-procedure reporting and payment methodology |
| 59 | Distinct procedural service | Coding edits and documentation supporting distinct services |
| 73 | Discontinued ASC procedure before anesthesia administration | Medicare ASC discontinued-procedure requirements |
| 74 | Discontinued ASC procedure after anesthesia or surgical start, as defined by policy | Documentation of circumstances and timing |
| 52 | Reduced services, when applicable | Whether the service was reduced rather than discontinued |
| RT | Right side | Laterality reporting when required |
| LT | Left side | Laterality reporting when required |
| FB | Device furnished without cost or with full credit | Device-credit adjustment policy |
| FC | Partial device credit | Device credit and payer requirements |

ASC TYPES & SURGICAL SPECIALTIES

ASC BILLING CHALLENGES
High-value cases mean every missed detail costs more.
The OR record may hold procedure, implant or supply details never reviewed at charge entry.
A procedure change or location discrepancy can interrupt payment.
Professional and facility claims follow separate reimbursement arrangements.
Some devices are packaged; others have separate or contract-specific payment.
A second procedure may be reduced or bundled.
One unresolved surgical claim can be a substantial balance.
CASE-TO-CLAIM VERIFICATION
ASC billing accuracy depends on what happens between the operating room and the claims department. We connect three records:
When these records disagree, we identify the discrepancy and coordinate clarification with facility staff. When an unpaid case needs further work, our denial management services handle corrections, appeals and payer follow-up.

BILLING SUPPORT COMPARISON
Outsourcing doesn’t mean your surgery center loses control of its financial operations.
It means assigning selected billing responsibilities to an outside team while you keep oversight of claims, payments and unresolved accounts.
| ASC billing requirement | In-house team | General billing provider | SkyLine MB ASC billing support |
|---|---|---|---|
| Facility vs. professional billing | Depends on staff expertise | May focus on physician claims | Facility claim review |
| Surgical charge capture | Requires coordinated case records | Varies by workflow | Case-based billing review |
| Implant and device reporting | Internal responsibility | May require specialist review | Device billing review |
| Multiple-procedure payment | Requires payer knowledge | General coding support | ASC payment rule review |
| Case-rate reconciliation | Requires contract information | Depends on service scope | Contract-based payment investigation |
| Surgical denial follow-up | Competes with daily operations | Varies by contract | Denial review and payer follow-up |
PAYERS, COMPLIANCE & SOFTWARE
Payment arrangements differ by payer, surgical specialty, procedure and facility. The ASC facility claim stays separate from surgeon, anesthesiologist and other professional claims.
Facility claims are subject to Medicare ASC payment rules, NCCI edits, multiple-procedure policies, modifier requirements and documentation standards. Discrepancies are flagged for correction or qualified coding review. SkyLine MB follows applicable HIPAA requirements, signs a Business Associate Agreement and uses role-based access.

ASC BILLING FAQS
ASC billing services manage insurance claims and reimbursement for covered outpatient surgical facility services. They can include eligibility verification, authorization support, surgical coding, charge capture, facility claim submission, payment posting, denial management and A/R follow-up.
ASC facility billing covers eligible facility services associated with outpatient surgery. Surgeons, anesthesiologists and other professionals generally bill separately for their professional services. Facility and professional claims follow different payment requirements.
Medicare uses the ASC Payment System for covered facility procedures and related services. Reimbursement depends on the procedure, payment indicators, current fee schedule, location adjustments and other Medicare policies.
Medicare maintains a list of ASC-approved procedures that changes through periodic updates. Eligibility must be verified using the code and payment files effective for the date of service.
Not always. Some implants and devices are included in the facility payment, while certain items have separate or contract-specific reimbursement. Billing depends on the procedure, device, payer and payment policy.
Medicare ASC facility services are paid under the ASC Payment System, while hospital outpatient departments generally use the Hospital Outpatient Prospective Payment System. The systems share some concepts, but their payment rules and rates aren’t interchangeable.
Under Medicare ASC rules, modifiers 73 and 74 identify qualifying discontinued procedures. Selection depends on whether anesthesia or the procedure had begun, along with policy definitions and documentation.
