SPECIALTY BILLING OVERVIEW
Orthopedic medical billing services handle insurance reimbursement for musculoskeletal evaluations, fracture treatment, surgical procedures, joint injections, imaging, orthotics and other covered orthopedic services.
What makes orthopedic billing challenging is the relationship between the initial service and the care that follows. Fracture treatment may include follow-up under a global package. Surgery may include routine postoperative care. A separately identifiable visit or additional procedure may be payable only when specific conditions are met — and braces, injections and imaging add further requirements.
SkyLine MB helps practices review these billing relationships so submitted claims reflect documented services and applicable payer rules.

Built around orthopedic care
From the first consultation to surgery and recovery, every claim needs the right billing treatment.
ORTHOPEDIC REVENUE CYCLE SUPPORT
Consider a patient who visits for knee pain, receives an injection, undergoes surgery several weeks later and returns for postoperative evaluations. Those encounters cannot all be billed the same way.
Some services are separately reportable. Others are included in the surgical package. Certain procedures need authorization, specific modifiers or supporting documentation — and a general billing workflow that misses those differences leads to delayed, denied or incorrectly paid claims.
SkyLine MB helps orthopedic practices manage the financial side of these treatment pathways. With 10 years of medical billing experience, we review clinical and billing information, coordinate claims, monitor payer responses and investigate unresolved balances.
Our focus isn’t how many claims leave your office. It’s whether documented care, reported charges and insurance payments are properly reconciled.
SPECIALTY REVENUE CYCLE SERVICES
Nine connected services for surgical, fracture, injection and DME claims.
01
Know the patient’s coverage before the financial questions begin.
We verify active insurance, relevant benefits, patient responsibility, network status and coverage requirements.
02
Joint replacement, spine procedures and advanced imaging may need prior approval.
We review approved codes, dates and facilities to catch discrepancies before processing.
03
The documented procedure determines the billing approach.
We review CPT coding, fracture care, global periods and modifiers — see the orthopedic code table.
04
Accurate claims need more than the correct patient name.
We prepare claims with provider identifiers, diagnoses, procedure codes and laterality, and correct rejections.
05
A surgical denial should have a clear next action.
We work authorization, medical necessity, coding edit and global period denials, with corrections or appeals where supported.
06
Don’t let unpaid surgical claims age without attention.
We monitor balances, review payer status and prioritize accounts that need action.
07
Make sure remittances tell the full financial story.
We post payments, reconcile adjustments and, with fee schedules or contracts, investigate underpayments.
08
New surgeons and locations bring new payer responsibilities.
We assist with credentialing, enrollment follow-up and CAQH; DMEPOS supplier enrollment is reviewed separately.
09
Clearer records make a growing practice easier to run.
We support patient balances, statements and reports on payments, denials and outstanding claims.
ORTHOPEDIC CODING REFERENCE
Different orthopedic procedures follow different coding rules. These examples show the range of services that appear in orthopedic claims.
| CPT code | Common service | Billing consideration |
|---|---|---|
| 99213–99215 | Established-patient office/outpatient E/M | Appropriate MDM or qualifying time |
| 20610 | Major joint or bursa aspiration/injection without ultrasound guidance | Procedure documentation and drug billing |
| 20611 | Major joint or bursa aspiration/injection with ultrasound guidance | Ultrasound documentation and retained image |
| 29075 | Application of a short-arm cast | Separately reportable only outside fracture-care rules |
| 29405 | Application of a short-leg cast | Treatment arrangement and global package |
| 25600 | Closed treatment of distal radial fracture without manipulation | Definitive fracture care and follow-up |
| 29881 | Knee arthroscopy with medial or lateral meniscectomy | Bundling and operative documentation |
| 29888 | Arthroscopically aided ACL repair/augmentation or reconstruction | Procedure coding and surgical documentation |
| 27130 | Total hip arthroplasty | Global surgical period and authorization |
| 27447 | Total knee arthroplasty | Medical necessity, global surgery and payer requirements |
Descriptions are simplified. Code selection and reimbursement depend on current CPT guidance, operative documentation, Medicare/NCCI edits and payer requirements. A code in this table is not always separately payable. CPT® is a registered trademark of the American Medical Association.
2026 GLOBAL SURGERY & FRACTURE BILLING
Global packages, fracture care and orthotics each decide what is — and isn’t — separately payable.
For many major orthopedic surgeries, the Medicare global period covers one preoperative day, the day of surgery and 90 postoperative days.
Routine postoperative care by the responsible practitioner is included, so not every visit after surgery is a separate E/M claim.
A splint at the initial encounter before referral is a different situation from closed fracture treatment with routine healing checks.
CMS NCCI guidance treats casting, splinting or strapping as part of definitive fracture treatment.
Knee braces, ankle orthoses and wrist supports can involve DMEPOS supplier enrollment, coverage criteria, orders and proof of delivery.
Not every device is separately payable, even when dispensed during an office visit.
Based on CMS global surgery policy, NCCI Policy Manual guidance on fracture care and casting, CPT surgical guidelines and Medicare DMEPOS requirements.
GLOBAL PERIODS & PROCEDURE MODIFIERS
Modifier usage must follow the actual service, provider arrangement, payer policy and clinical documentation. A modifier can’t be added just to get bundled services paid.

| Modifier | What it identifies | Why it matters |
|---|---|---|
| 24 | Unrelated E/M during a postoperative period | Separately identifiable unrelated care |
| 25 | Significant, separately identifiable E/M on the day of a procedure | Supported additional E/M |
| 57 | Decision for major surgery | Qualifying decision-to-operate E/M |
| 58 | Staged or related procedure during the postoperative period | Planned, more extensive or therapeutic follow-up procedure |
| 78 | Unplanned return to the OR for a related procedure | Qualifying related surgical treatment |
| 79 | Unrelated procedure during the postoperative period | Separate procedure unrelated to the original surgery |
| 54 | Surgical care only | Transfer or division of surgical care |
| 55 | Postoperative management only | Documented transfer of postoperative care |
| RT / LT | Right or left side | Laterality requirements |
| 59 | Distinct procedural service when allowed | Appropriate separation under coding edits |

COMMON ORTHOPEDIC BILLING PROBLEMS
Treatment pathways create billing questions that routine claim processing misses.
A follow-up may fall within the global period — or be an unrelated service needing documentation.
Definitive fracture treatment may already include the cast or splint.
DMEPOS claims need supplier status, coverage criteria, orders and delivery proof.
Procedure code, drug, guidance, diagnosis or laterality affect payment.
Changes between planned and completed surgery affect coverage.
Workplace and accident injuries involve different claim administrators.
ORTHOPEDIC CLAIM READINESS
Orthopedic claims usually reflect a treatment process, not a single appointment. Before submission we connect three records:
When these records disagree, we identify the issue and coordinate clarification before submission or during denial follow-up. Consistent medical coding review helps catch unsupported combinations and avoidable discrepancies.

ORTHOPEDIC BILLING COMPARISON
You may already have a capable internal billing department.
The question is whether your practice has enough time and specialist support to manage surgery packages, fracture treatment, injections, DME and unresolved claims consistently.
| Billing requirement | In-house team | General billing provider | SkyLine MB orthopedic support |
|---|---|---|---|
| Surgical global periods | Depends on staff knowledge | May need specialist review | Global-period and modifier review |
| Fracture care claims | Internal coding responsibility | General procedure coding | Fracture-treatment billing review |
| Joint injections | Procedure/drug reconciliation | Varies by service scope | Injection and drug-claim review |
| Braces and orthotics | DMEPOS managed internally | May need separate resources | Orthotic billing requirement review |
| Surgical denials | Competes with daily workload | Depends on vendor | Denial investigation and follow-up |
| Outstanding A/R | Internal staff responsibility | Contract-dependent | Aging and payer follow-up |
PAYERS, COMPLIANCE & SOFTWARE
Orthopedic claims are shaped by global surgical packages, NCCI edits and code-specific reporting limits. Braces, splints and orthotics may be subject to coverage, supplier enrollment, ordering and delivery documentation rules.
Claims are billed only for services actually performed and documented; global surgery and NCCI questions are routed to qualified coding professionals or your staff. SkyLine MB follows applicable HIPAA requirements, signs a Business Associate Agreement and uses role-based access.

ORTHOPEDIC BILLING FAQS
Orthopedic billing services manage insurance claims and reimbursement for orthopedic consultations, fracture treatment, surgical procedures, injections, orthotics and related covered services. They may include coding review, authorization support, claim submission, denial management, payment posting and accounts receivable follow-up.
A global surgical period is the payment period associated with a procedure during which certain preoperative, operative and postoperative services are included in the surgical payment. Medicare uses different global indicators, including 0, 10 and 90 days, depending on the procedure.
Some postoperative services are included in the global surgical package. Separately identifiable unrelated E/M services, staged procedures or qualifying additional procedures may be billable when specific requirements are met.
Fracture billing depends on the treatment performed, whether definitive care was provided, the use of manipulation, applicable global packages and who provides follow-up care. Casting or splinting may be included in definitive treatment rather than separately payable.
Modifier 58 identifies an eligible staged, more extensive or therapeutic procedure performed during the postoperative period. The procedure and documentation must satisfy applicable coding requirements.
Modifier 78 is used for a qualifying unplanned return to the operating or procedure room for a related procedure during the postoperative period. Modifier 79 identifies an unrelated procedure during the postoperative period.
Certain braces and orthotic devices may qualify for separate reimbursement, but payment depends on applicable coverage, DMEPOS supplier requirements, device coding and supporting documentation.
Joint injection billing depends on the joint treated, whether ultrasound guidance was used, the documented procedure, payer policies and any separately reportable medication. CPT 20610 and 20611 are common examples for major joint or bursa procedures.
