CHIROPRACTIC BILLING, EXPLAINED
Chiropractic medical billing services manage the insurance claims and reimbursement process for spinal manipulation and other eligible services provided by chiropractic practices.
The difficulty often lies in what a payer covers. Medicare limits chiropractic coverage to qualifying manual manipulation of the spine to correct a subluxation. Commercial plans may recognize more services but impose visit limits, medical necessity requirements or other restrictions.
CPT 98940, 98941 and 98942 describe manipulation of different numbers of spinal regions, and the code must match the documented regions treated — while Medicare active treatment claims need the AT modifier. SkyLine MB helps chiropractors review these details, prepare claims and investigate reimbursements that need attention.

Billing that understands chiropractic care
From adjustment documentation to insurance follow-up, your claims deserve more than routine processing.
CHIROPRACTIC REVENUE CYCLE SUPPORT
Chiropractic practices rely on recurring visits, which makes small billing problems easy to overlook. A patient may begin active corrective treatment, improve and eventually move to maintenance care. Clinically, care continues. Financially, the insurance treatment of those visits can change.
A Medicare claim for qualifying active manipulation isn’t handled like a maintenance visit. A commercial payer may cover extraspinal manipulation while Medicare doesn’t. Another insurer may require documentation for continued treatment or cap covered visits.
SkyLine MB connects treatment records, coverage information, procedure codes and payer responses. With 10 years of medical billing experience, we review billing information, submit claims, resolve administrative errors and follow up on unpaid balances.
More visits are good for a busy practice. Better visibility into which visits are payable is good for its revenue cycle.

CHIROPRACTIC CODING REFERENCE
Correct chiropractic coding begins with the documented service and the number of qualifying spinal regions treated.
| CPT code | General service | Main billing consideration |
|---|---|---|
| 98940 | Chiropractic manipulative treatment, spinal, 1–2 regions | Regions documented and Medicare active treatment rules |
| 98941 | Chiropractic manipulative treatment, spinal, 3–4 regions | Accurate region count and medical necessity |
| 98942 | Chiropractic manipulative treatment, spinal, 5 regions | Documentation supporting five spinal regions |
| 98943 | Chiropractic manipulative treatment, extraspinal, 1+ regions | Not covered under the Medicare chiropractic benefit |
| 99202–99205 | New-patient office/outpatient E/M | Payer coverage and provider eligibility |
| 99212–99215 | Established-patient office/outpatient E/M | Payer coverage and documented service |
| 97110 | Therapeutic exercises | Payer coverage and eligible rendering provider |
| 97140 | Manual therapy techniques | Payer-specific provider eligibility and service rules |
| 97012 | Mechanical traction | Payer-specific coverage and billing requirements |
Listing a code does not mean Medicare covers it when performed or ordered by a chiropractor — Medicare’s chiropractic benefit is limited to qualifying spinal manipulation. Other services may be payable under commercial or other arrangements, subject to provider eligibility and documentation. CPT® is a registered trademark of the American Medical Association.
MEDICARE RULES THAT MATTER
Medicare covers a specific chiropractic benefit — not every treatment a clinic offers.
Active or corrective manipulation billed with 98940, 98941 or 98942 needs the AT modifier when applicable.
AT alone doesn’t prove medical necessity, and it must never be used to get maintenance visits paid — Medicare doesn’t cover maintenance therapy.
Medicare requires documentation supporting the spinal subluxation and medically necessary manipulation.
That can include exam findings, the regions treated, the treatment plan and progress notes. We flag billing-related gaps for the chiropractor to complete.
CPT 98943 (extraspinal) isn’t covered under Medicare’s chiropractic benefit, though other payers may differ.
X-rays, exams, massage and therapies furnished or ordered by a chiropractor aren’t Medicare-payable just because they relate to spinal care.
Based on the Medicare Benefit Policy Manual chiropractic services chapter, CMS AT modifier guidance, CPT chiropractic manipulative treatment guidelines and commercial payer policies.
COVERAGE & MODIFIER REVIEW
ABN-related modifiers aren’t interchangeable, and GP doesn’t turn a noncovered chiropractic service into a covered Medicare therapy benefit. The right modifier depends on the actual circumstances and payer rules.

| Modifier / detail | Purpose | What to check |
|---|---|---|
| AT | Active/corrective treatment on applicable Medicare spinal manipulation claims | Documentation supports active treatment, not maintenance |
| GA | Required waiver of liability (ABN) statement on file | Valid ABN circumstances and current Medicare rules |
| GY | Statutorily excluded service or not a Medicare benefit | Correct exclusion and reporting circumstances |
| GZ | Expected denial as not reasonable and necessary, no valid ABN | Appropriate usage and claim circumstances |
| GP | Services under a physical therapy plan of care | Qualified provider, payer policy and actual service |
| 25 | Significant, separately identifiable E/M on a procedure day | Documentation and payer permission |
| Diagnosis sequencing | Condition supporting the billed service | Current ICD-10-CM guidance and payer necessity rules |
| Spinal regions | Determines CMT code selection | Documentation supports 1–2, 3–4 or 5 regions |

COMMON CLAIM PROBLEMS
Recurring visits make small billing errors add up quickly.
Active treatment claims can deny without AT; AT on maintenance care is a compliance problem.
98941 shouldn’t be billed when the record supports only one or two regions.
Some plans limit chiropractic visits or exclude services entirely.
Treatment status matters under Medicare’s limited benefit.
Commercial plans apply specific rules to manual therapy and exercises.
Unpaid visit balances become a real burden when follow-up falls behind.
VISIT-TO-CLAIM VERIFICATION
The patient came in, treatment was provided and the visit was documented. Before submission, the billing still has to answer three questions:
SkyLine MB identifies inconsistencies between these records and coordinates corrections. Accurate eligibility verification also separates covered treatment from patient-responsibility services before billing questions arise.

|---|---|---|---|
| Spinal manipulation codes | Depends on coding knowledge | General CPT review | CMT region and coding review |
| Medicare AT modifier | Managed internally | May need specialty guidance | AT and treatment-status review |
| Visit limits and coverage | Front-desk and billing workload | Depends on service scope | Chiropractic benefit verification |
| Accident-related accounts | Separate admin processes | Varies by vendor | Scope-based accident claim coordination |
| Chiropractic denials | Internal payer follow-up | Contract-dependent | Denial investigation and correction |
| Outstanding A/R | Regular staff attention | Varies by agreement | Aging review and payer follow-up |
PAYERS, COMPLIANCE & SOFTWARE
Chiropractic reimbursement differs substantially between Medicare and commercial insurance. Medicare’s benefit is limited to qualifying manual manipulation of the spine to correct a subluxation, with active treatment documentation when AT applies.
Claims are billed only for services actually performed and documented; documentation gaps are routed to the treating chiropractor. SkyLine MB follows applicable HIPAA requirements, signs a Business Associate Agreement and uses role-based access.

Medicare does not pay chiropractors for diagnostic X-rays under its limited chiropractic benefit, even when imaging is used in evaluating a spinal condition.
No. Extraspinal chiropractic manipulative treatment reported with 98943 is not covered under Medicare’s chiropractic benefit. Other insurance plans may have different rules.
