Untimed and timed SLP codes billed correctly.
SPEECH THERAPY BILLING, EXPLAINED
Speech therapy billing services manage the insurance and reimbursement process for speech-language pathology evaluations, communication therapy, swallowing treatment and other covered SLP services.
Unlike general medical billing, SLP claims require attention to evaluation versus treatment codes, untimed procedures such as 92507, payer authorization requirements, the GN modifier and documentation supporting the services performed.
For many practices the real challenge isn’t the first claim — it’s resolving everything that happens when the claim is delayed, denied or paid incorrectly. SkyLine MB manages those steps, from verification and charge review to denial resolution and A/R follow-up.

SKYLINE MB / SPEECH THERAPY BILLING
A full appointment schedule should support your practice financially. Yet speech-language pathologists often face payment delays even when services are completed and properly documented.
One payer may require authorization before language therapy begins. Another may apply different coverage rules for developmental speech disorders. A Medicare claim may need the GN modifier, while a swallowing treatment claim may need additional documentation to support medical necessity.
At SkyLine MB, we review therapy benefits, monitor authorization details, prepare claims, investigate denials and follow up on outstanding payments. With 10 years of medical billing experience, we support independent SLPs, pediatric therapy clinics and rehabilitation practices across the United States.
Your therapists shouldn’t have to become insurance follow-up specialists to keep your practice running.
WHAT WE HANDLE
Nine connected services that help SLP practices manage claims from patient eligibility through final account reconciliation.
01
Know what’s covered before the session becomes an unpaid claim. Coverage can vary by diagnosis, plan, age and treatment requirements.
We review active coverage, SLP benefits, patient responsibility, referral requirements and visit limitations.
02
Speech therapy often involves recurring appointments, and many payers limit covered visits.
We track approved sessions, effective dates and remaining visits, and coordinate the information needed for reauthorization.
03
We review codes such as 92507, 92508, 92521–92524, 92526 and 92610 — see the SLP CPT code table.
Common SLP codes are not all time-based, so claims follow code definitions and payer rules, not a 15-minute assumption.
04
Missing GN modifiers, incorrect patient details, unsupported diagnosis combinations and authorization discrepancies can delay SLP claims.
We prepare electronic claims, monitor responses and correct rejections quickly.
05
Denials can come from coverage limits, missing authorizations, medical necessity concerns, incorrect codes or incomplete documentation.
We review each denial, coordinate corrections, reconsiderations or appeals, and track recurring patterns.
06
As practices grow, outstanding balances become hard for internal staff to manage.
We review claim aging, follow up with payers, investigate payment status and track every unresolved account.
07
Know which sessions were paid and which need review. We post payments from ERAs and EOBs, record adjustments and patient balances.
Potential underpayments and account differences are flagged for follow-up.
08
Adding another SLP shouldn’t leave enrollment paperwork behind.
We assist with credentialing administration, CAQH information, payer applications and enrollment follow-up. Network participation remains subject to payer approval.
09
Families and patients have questions about deductibles, copays and balances. We support statements, balance tracking and billing inquiries within the agreed scope.
Reports show payments, denials and outstanding balances.

MEDICARE SLP BILLING UPDATES
Plan-of-care requirements, procedure-specific coding and modifiers that set speech-language pathology apart from other therapy disciplines.
For 2026, the KX threshold is $2,480 for PT and SLP combined — not a separate amount per discipline.
It is not a cap. Above it, KX confirms continued care is medically necessary and documented.
Common treatments such as 92507 and 92526 are generally untimed and reported per session.
Billing extra units for every 15 minutes creates incorrect claims.
Medicare requires GN on services furnished under an SLP plan of care.
Documentation should support the patient’s condition, goals, skilled services and continued need for care.
Based on CMS outpatient therapy guidance (Medicare Benefit Policy Manual Ch. 15) and the CY 2026 Physician Fee Schedule. Commercial and Medicaid payers may apply different rules.
SLP MODIFIERS & PLACE OF SERVICE
Missing or incorrect modifiers change how insurers process SLP claims. Usage depends on the procedure, setting, payer policy, documentation and date of service.

| Modifier / POS | Purpose | Billing risk |
|---|---|---|
| GN | Identifies services under an SLP plan of care | Missing required therapy modifier |
| KX | Medical necessity met at or above the therapy threshold | Missing when required or unsupported by records |
| 52 | Reduced services, when applicable | Reported without proper reduced-service indication |
| 59 | Distinct procedural service, when permitted and supported | Improper unbundling of services |
| 95 | Synchronous telemedicine service, when applicable | Not accepted or not required by the specific payer |
| POS 02 | Telehealth provided other than in the patient’s home | Incorrect telehealth location reporting |
| POS 10 | Telehealth provided in the patient’s home | Incorrect place-of-service selection |

WHO WE SERVE
SLP practices serve patients with different communication, feeding and swallowing needs, and billing requirements differ by age, setting and payer. We support:
Whether you see adults, children or both, your billing workflow should fit the way your practice delivers care.
COMMON CHALLENGES
Small coding and authorization gaps add up quickly in a recurring-visit specialty.
Applicable Medicare SLP claims require GN. Missing or incorrect modifiers create processing issues.
Evaluations and treatment sessions use different codes. Wrong code selection affects payment.
Patients may keep attending after the authorized visit limit or approval period ends.
Medicaid and managed care requirements vary by state, plan, provider type and service.
Insurers may ask for more information supporting evaluation or treatment services.
Claims can stay unpaid because of added processing requirements or incomplete follow-up.
AUTHORIZATIONS & VISITS
Your team may deliver weeks of treatment before discovering an authorization expired. We align three parts of the billing process:
Reviewing these records together catches discrepancies before submission, and we coordinate reauthorization follow-up with your administrative team.

IN-HOUSE VS. OUTSOURCED

SLP BILLING FAQS
Speech therapy billing services support insurance reimbursement for speech-language pathology evaluations, treatments, swallowing services and other covered procedures. They may include benefits verification, coding review, claim submission, denial management, payment posting and A/R follow-up.
Common codes include 92507 for individual speech-language treatment, 92508 for group treatment, 92521–92524 for speech-language evaluations, 92526 for swallowing treatment and 92610 for swallowing function evaluation. Coding depends on the actual service performed.
The GN modifier identifies services delivered under a speech-language pathology plan of care. Medicare generally requires it on applicable outpatient SLP services.
No. CPT 92507 is generally an untimed, session-based service and should not be billed in extra units for each 15-minute increment. Payer policies should still be reviewed.
