GYNECOLOGY BILLING, EXPLAINED
Gynecology billing services manage insurance reimbursement for preventive examinations, problem-oriented gynecologic visits, diagnostic procedures, contraceptive services and gynecologic surgery.
The complexity often comes from the relationship between services performed during the same encounter. A patient may receive preventive care and also need evaluation of an unrelated concern. A procedure may include services that can’t be billed separately. A contraceptive device and its insertion may require different claim lines, depending on payer policy and who supplied the device.
That makes gynecology medical billing more involved than routine office visit processing. SkyLine MB helps practices review those details, prepare properly supported claims and investigate insurance payments that need attention.

Billing that understands women's health
From preventive visits to procedural claims, each service deserves the correct billing approach.
GYNECOLOGY REVENUE CYCLE SUPPORT
Consider a typical day at your practice. One patient arrives for her annual examination. Another needs a colposcopy after an abnormal screening result. Someone else returns for contraceptive management, while a surgical patient needs follow-up care.
Your clinicians understand the difference between these encounters. Your billing process needs to understand it too.
At SkyLine MB, we help translate documented gynecologic care into claims that reflect the services performed and each payer’s requirements — whether a visit was preventive or problem-oriented, which procedures are separately reportable, which modifiers apply and how reimbursement is tracked. With 10 years of medical billing experience, we focus on reducing avoidable administrative friction.
Your schedule shows how busy your practice is. Your collections show how the business is performing. We help connect the two.
WHAT WE HANDLE
Nine connected services built around preventive, procedural and surgical gynecology claims.
01
Preventive and diagnostic benefits may not work the same way.
We verify active insurance, women’s health benefits, patient responsibility and coverage requirements before claims are processed.
02
Some gynecologic surgeries, diagnostic procedures and facility-based services require prior authorization.
We review procedure codes, service locations and effective dates, and coordinate outstanding requirements.
03
What occurred during the appointment determines what can be billed.
We review preventive and problem-oriented services, office procedures, surgery codes and modifiers — see the gynecology code table.
04
Incomplete claims create unnecessary delays.
We prepare claims with the right patient, provider, procedure, diagnosis and insurance information, and correct rejections for resubmission.
05
A denial may come from a modifier issue, missing authorization, bundling edit, eligibility problem or documentation concern.
We investigate payer responses and coordinate corrections or appeals.
06
Older gynecology claims still deserve attention.
We organize outstanding balances, investigate unresolved reimbursements and follow up within applicable filing requirements.
07
We reconcile remittances, contractual adjustments and patient balances.
When payer contract information is available, we review differences that may need further investigation.
08
Adding providers shouldn’t create avoidable billing interruptions.
We assist with payer enrollment, CAQH updates, provider information and credentialing follow-up.
09
We support statements and payment tracking within the agreed scope.
Reports cover claim activity, denials, outstanding balances and collections.
COMMON GYNECOLOGY CPT & HCPCS CODES
One practice may bill preventive examinations, office-based procedures and complex surgery. Accurate coding requires the medical record to support every service reported.
| Code | Service | Billing consideration |
|---|---|---|
| 99385 | New-patient preventive visit, age 18–39 | Age and patient status |
| 99395 | Established-patient preventive visit, age 18–39 | Preventive service documentation |
| 99213 | Established-patient office/outpatient E/M | Appropriate MDM or time |
| G0101 | Medicare screening pelvic and clinical breast examination | Medicare coverage and eligibility |
| Q0091 | Collection, preparation and conveyance of screening Pap smear specimen | Screening criteria and payer policy |
| 57452 | Cervical colposcopy | Procedure performed and coding edits |
| 57454 | Cervical colposcopy with biopsy and endocervical curettage | Documentation of both components |
| 58100 | Endometrial sampling by biopsy | Procedure documentation |
| 58300 | IUD insertion | Procedure and applicable device billing |
| 58301 | IUD removal | Procedure documentation and payer policy |
| 58558 | Operative hysteroscopy with endometrial sampling and/or polypectomy | Procedure details and bundling |
| 58571 | Total laparoscopic hysterectomy, uterus 250 g or less, with removal of tube(s) and/or ovary(s) | Surgical approach and operative documentation |
Descriptions are simplified. Preventive codes vary by age and patient status, surgical coding depends on operative details, and payer rules determine coverage and reimbursement. Check current CPT and HCPCS references before submission. CPT® is a registered trademark of the American Medical Association.
2026 GYNECOLOGY CODING & REIMBURSEMENT
The right code matters. The relationship between codes matters just as much.
A preventive appointment and a visit for a specific concern aren’t interchangeable.
When a significant, separately identifiable problem-oriented E/M is performed at the same encounter, separate billing may be appropriate if requirements are met.
Colposcopy with biopsy, with endocervical sampling, or with both may need different codes — overlapping codes trigger bundling edits.
An IUD insertion and the device itself may involve separate billing, depending on payer and who supplied it.
Gynecologic surgery claims follow global surgical package rules.
Routine postoperative care is included in the surgical payment; other services qualify for separate reporting only when the criteria are met.
Based on CPT preventive medicine and E/M guidelines, Medicare screening rules for G0101/Q0091, NCCI policy and CMS global surgery rules. Payer policies vary by plan.
PROCEDURE & E/M MODIFIERS
Not every modifier applies to every gynecology service. Modifier selection must reflect the actual care — and can’t be used simply to bypass a denial.

| Modifier | Purpose | What to review |
|---|---|---|
| 25 | Significant, separately identifiable E/M on the day of a procedure or other service | Whether additional E/M work is supported |
| 59 | Distinct procedural service, where appropriate | Documentation and coding-edit restrictions |
| XE | Separate encounter | Whether services occurred during distinct encounters |
| XS | Separate structure | Whether a separate anatomical structure supports reporting |
| 51 | Multiple procedures, when applicable | Payer-specific multiple procedure rules |
| 57 | Decision for major surgery | Timing and documentation of the decision |
| 78 | Unplanned return to the OR for a related procedure during the postoperative period | Applicable global surgical rules |
| 79 | Unrelated procedure during the postoperative period | Documentation showing an unrelated procedure |

PRACTICES & PROCEDURES WE SUPPORT
We don’t rely on a one-size-fits-all process. Our gynecology billing support covers:

COMMON BILLING PROBLEMS
Most lost revenue comes from a small set of repeatable mistakes.
Preventive care and diagnostic evaluation follow different coding and coverage rules.
IUD placement or endometrial biopsy can involve details a routine visit doesn’t.
An E/M billed with a procedure may be rejected as not separately reportable.
Multiple reported procedures can overlap under coding rules.
Global surgery rules, multiple-procedure reductions or contracts can change payment.
Unresolved claims get harder to pursue as payer deadlines approach.
VISIT-TO-CLAIM VERIFICATION
A gynecology appointment can involve more than one category of care. We connect three essential pieces of information:
When the information doesn’t agree, we identify the discrepancy and coordinate the next action. For planned procedures that need payer approval, our prior authorization services handle the administrative work.

Your practice may already have capable administrative staff. Outsourcing adds support for the tasks that need more time, specialty billing attention or consistent insurance follow-up.
The comparison shows where gynecology-specific support makes a difference.
| Billing priority | In-house team | General billing provider | SkyLine MB GYN billing support |
|---|---|---|---|
| Preventive vs. problem visits | Depends on staff experience | General E/M processing | Gynecology-specific visit review |
| Same-day E/M and procedures | Internal coding review | Depends on service scope | Documentation and modifier assessment |
| IUD procedure and device claims | Internal responsibility | May require additional review | Procedure and eligible device charge review |
| Colposcopy and biopsy claims | Requires procedure coding knowledge | General surgical coding | Procedure and bundling review |
| Surgical denials | Staff follow-up | Varies by vendor | Investigation and appropriate appeals |
| Outstanding A/R | Competes with daily duties | Depends on contract | Aging review and payer follow-up |
PAYERS, COMPLIANCE & SOFTWARE
Reimbursement depends on the patient’s plan, the service performed, provider enrollment and payer-specific rules. Coverage, frequency limits and diagnosis requirements are reviewed for screening and procedures.
Gynecologic procedures can be subject to NCCI edits, global surgery rules and documentation requirements; discrepancies are flagged for correction or review by a qualified coding professional. SkyLine MB follows applicable HIPAA requirements, signs a Business Associate Agreement and uses role-based access.

GYN BILLING FAQS
Gynecology billing services manage insurance claims and reimbursement for preventive women’s health visits, diagnostic procedures, contraceptive services and gynecologic surgery. They may include eligibility verification, coding review, claims submission, denial management, payment posting and accounts receivable follow-up.
Gynecology billing requires attention to preventive versus problem-oriented visits, screening coverage, office procedure coding, contraceptive devices, surgical bundling and payer-specific requirements. These affect whether multiple services from the same encounter are separately payable.
They may be separately reportable when a significant, separately identifiable problem-oriented service is performed and documented, subject to payer policy. The additional service shouldn’t be billed merely because a patient raised a concern during a preventive examination.
Modifier 25 identifies a significant, separately identifiable E/M service performed by the same provider on the day of a procedure or other service. Its use must be supported by documentation and applicable coding requirements.
CPT 58300 is commonly used for IUD insertion and 58301 for IUD removal. Device billing may involve separate HCPCS codes depending on the product, payer and billing arrangement.
No. Separate reimbursement depends on the patient’s coverage, services performed, applicable codes and payer policy. Medicare uses specific HCPCS codes such as G0101 and Q0091 in eligible circumstances.
Yes. We support colposcopy and biopsy claims, including procedure coding review, relevant coding edits, denial investigation and insurance follow-up. The reported codes must reflect the procedures documented.
Some procedures do, particularly certain hospital-based, outpatient or elective surgeries. Requirements vary by payer, plan, procedure and facility.
Yes. We support individual gynecologists, women’s health clinics and multi-provider practices, with workflows based on their services and administrative needs.
We support accounts receivable review, denial investigation, payer follow-up and appropriate corrections or appeals. Payment recovery depends on eligibility, documentation, filing deadlines and payer decisions.
We assist with credentialing administration, payer enrollment applications, CAQH information and provider record updates within the agreed service scope.
Pricing depends on the number of providers, claim volume, payer mix, procedure complexity and services required. Contact SkyLine MB for a free consultation and a customized proposal.
